Supported self-management: peer support guide
Supported self-management means increasing the knowledge, skills and confidence a person has in managing their own health and care by putting in place interventions such as: peer support, self-management education and health coaching.
The guide explains what peer support is, why it is important and what good peer support looks like. It also describes models of peer support and how to signpost and refer people to peer support, and highlights the considerations when setting up or commissioning peer support.
